**Job Summary**:
Since early December 2021 ABC have led the way in mobilizing the COVID-19 vaccination programme and have delivered over 100 000 vaccinations to date. Ensuring equity of access is a core part of ABC’s values and we have ensured vaccine delivery to those who may not access major sites or use online booking since the start of the program. As we have progressed through the cohorts, we have identified multiple disadvantages the cohorts need. our need from delivering vaccines have since taken more targeted approach to address health inequalities and provide NHS health care checks and Equity wellbeing checks and we are looking to appoint an equity engagement advocate to support this essential work, building on the trust and attract new cohort by providing excellent engagement techniques.

We are building on the learning from the vaccine programme to pilot a new mobile vehicle, which forms part of the existing Crawley Programme. This unit will go out to the streets of Crawley offering cardiology and respiratory testing, with a view to reach those in the most deprived areas of Crawley, and increase screening uptake, helping the Crawley population live well for longer.

You will be driving the Mobile Van from ABC’s base in Horley to a weekly set location daily. Upon arrival you will be responsible for setting the van up, ie. putting the staircase up and unloading the van. The SASH team will meet you at the location and will be using the van to deliver their respiratory and cardiology service. Your role will be to engage with booked patients and also reach out to local vulnerable communities, such us homeless and promote this service and the equity services.

The post is likely to involve some evening and weekend working to support outreach sessions, hours will need to be worked flexibly but overtime will be paid for anything over 37.5 hours per week.

**Main Duties and Responsibilities**

At Outreach Sessions
- Responsible for driving the van to a pop-up location on time.
- Unloading and setting up the van on arrival and packing back up at the end of the shift.
- Keeping all equipment needed in good condition.
- Check equipment is in good working order at start of the shift and at end of the shift, ready for next working day.
- Responsible for reporting any damaged or replacement materials needed to relevant equity coordinator.
- Administrative support to smooth running of session and filling any preforms.
- Using clinical systems and Microsoft teams online platforms to record information.
- Engagement support with targeted population to increase service uptake.
- Identifying eligible cohorts according to inclusion and exclusion criteria.
- Signposting eligible cohort to Equity Clinicians and exchanging any relevant info.
- Escalation of any patient's health issues to Equity Clinician present.
- Escalation of any issues to manager.
- Ensure all patient information is recorded appropriately in line with patient wishes.

Engagement and Advocacy
- Engage with the cohort identified as vulnerable and at risk of multiple disadvantages.
- With permission offer support and list all health inequalities of the targeted individual, offer to follow up with sign posting material.
- Inform Coordinators who will be able to record any support provided, on the patient’s health record.
- Inform service users of any contact made with services.

- Keep records of sessions for audition and input all data into excel workbook.
