Overview:
**Change Grow Live** are a charity dedicated to the belief that we can make a difference to our Service Users lives, offering support and respect in a safe environment, treating each user as an individual and working with them to find the right treatment and care options.

We have an exciting opportunity for a
**Navigator Outreach Worker** who has the skills and experience to assess and engage with our service users, ensuring that they access the right intervention at the right time, in a way that best meets their needs.

You will focus on engaging with people who have a history of rough sleeping and substance misuse and provide support to clients identified as being at risk of eviction or having a history of their accommodation breaking down quickly, resulting in individuals returning to the street.

No two days are the same and we treat every person who comes into our service as an individual, so being flexible, curious and a good listener are key.

**Hours**: 37.5 hrs per week, 15 month fixed term contract

**Salary**: £26,393.77 - £30,316.74 dependent on experience

**Responsibilities**:
**About the role**:

- To work with an identified number of clients who are at risk of abandoning or eviction from their accommodation
- To identify and establish contact with long-term rough sleepers, including those unwilling to engage with services, providing assertive outreach
- To work proactively to ensure that service users have access to ongoing healthcare
- To work closely with all partner services in Brighton and Hove, sharing appropriate information and ensuring that action plans are carried out

**About you**:

- Understanding of the needs of people who are rough sleeping or insecurely housed
- Understanding the importance of multi-agency working
- Be an excellent communicator, both verbal and written with good IT skills
- Proactive and able to work on own initiative with excellent time management and prioritising skills
- Have a good working knowledge of mental health interventions, services, and good practice
- Be a supportive team player with strong interpersonal skills with the ability to work in partnership with a wide range of professionals, agencies, and internal/external stakeholders
- Ability to manage change successfully in a way that prioritises the needs of service users
- Experience of, and a commitment to, working positively in partnership with a range of statutory and voluntary agencies
