Health Engagement Worker

**Job purpose Patient focus**: Facilitating patient engagement through face to face and telephone interactions, providing follow-up between sessions to support patient recovery. Project focus: Providing logístical support to the delivery of the project including all practical and administrative aspects. Partner focus: Working closely with the Project Coordinator to deliver clinical outreach in Brighton & Hove, ensuring strong integration into the wider homeless health network. Core duties Supporting individual patients to engage with the service, this will include face to face and telephone interactions.
Advocacy in registering patients for medical and support services and assisting them in addressing all aspects of physical and mental health or substance addiction needs, as well as advocating for them to get access to housing and social security benefits. Providing logístical support to the project, for example collecting medications or accompanying patients to follow-up appointments. Designing and delivering health promotion sessions, for example to improve vaccine or cervical screening uptake. Liaising with partners to arrange and plan outreach sessions.
Advocacy for patients. Building partnerships with other services. Use of IT systems for record keeping. Engaging in learning and development exercises relating to the project as it develops.
Working with the wider Arch team. Contributing to report writing. Working to support the Outreach Team Coordinator with all aspects of the project General Responsible for checking the accuracy of work and maintaining a strong attention to detail at all times. Responsible for personal development with support from line manager.
