An excellent opportunity has arisen within our Acute and Integrated Care Community Services at East Cheshire NHS Trust. We are looking to recruit an enthusiastic and compassionate Bank Trainee Community Advanced Clinical Practitioner to join our crisis response team

The role will undertake advanced clinical assessment of patients with differential and undiagnosed illness or exacerbation of existing conditions using advanced clinical skills, critical thinking, decision making and risk analysis to provide safe, high quality, evidence-based care preventing unnecessary acute hospital admissions.

Provision of advanced practice support to members of the multi-professional team through provision of professional advice, consultancy, clinical expertise and leadership to patients, carers and staff. This will be done in collaboration with senior colleagues within the team and will require demonstration of advanced skills and competencies to influence the delivery of programmes of care in the management of patients

To work alongside the Care Community Coaches, Clinical Leads, and Specialist Consultants to work strategically, contributing to the planning and development of integrated services, acting as a driver for change and ensuring standards of care and best practice in line with up-to-date evidence base.

Key Duties/Responsibilities

**Clinical**:
1. Deliver evidence-based personalised and compassionate care which places quality at the heart of specialist practice. Care will be responsive to individual patient needs and reflect current local protocols and national guidelines.
2. To work flexibly in role to support and respond to variations in demand across Community services
3. Within the scope of professional practice to work to acquire skills to be able to independently undertake holistic (physical, psychological and social) assessment and examination making critical clinical decisions based on advanced clinical skills assessment in line with evidence based practice, legislation policies and procedures

**Leadership/Management**:
1. Develop a visible, clinical presence/leadership as a tACP acting as a positive role model across professional and organisational
2. Develop skills and work collaboratively having both advisory and clinical input into patient care across the service, offering appropriate support and advice to all professions on patient care and management.
3. To work across the multi-professional services in Community Care teams to develop skills to support timely patient assessment, diagnosis, treatment and where appropriate referral on / signpost to other services to ensure a good patient experience.
