An exciting opportunity has arisen to work within the acute oncology team as a Macmillan Cancer Care Co-ordinator. We are looking for a highly motivated, enthusiastic and caring individual to work within cancer care. The role has been developed in order to provide holistic needs assessments to cancer patients, working primarily with the acute oncology team but also with other cancer specialist teams within the hospital.
The acute oncology service provides specialist support to patients requiring urgent care due to complications of their cancer or treatment. This role will support both the nursing teams and patients throughout their pathway ensuring timely management.
It is therefore essential that you possess excellent communication, organisational and IT skills as well as having a good understanding of patient centred care.
This post offers an exciting opportunity to support the senior nursing team within the acute oncology service that will not only impact on the quality of patient care but will be challenging and rewarding on a personal level.
Follow up patient enquiries on a day-to-day basis liaising with the acute oncology nurse consultant/nursing team.
Use good communication skills, basic clinical awareness and utilise appropriate tools and procedures. Refers complex decisions regarding patients to specialist nursing team for assessment and review.
Daily use of systems to document/monitor all aspects of patient care coordination. Support data collection for service delivery, audit, and quality surveillance.
In conjunction with nurse consultant will discuss and coordinate the care for a defined group of patients. Organise and prioritise the designated workload in relation to identified needs.
Support the nurse consultant in the collection of appropriate information to complete the holistic needs assessment documentation for a wide range of cancer patients.
Communicate effectively to ensure safe and effective transition of patient care between services and relevant information shared.
Support the nurse consultant in delivering patient-centred, self-management support/education to non-complex patients, including how to self-assess (an understanding of what signs, symptoms, or situations to be aware of that would indicate concern).
Promote telephone advice line for patients/carers to use when they feel that their condition or needs have changed, in and out of hours. Act as point of contact for patients/carers, signposting all enquiries where appropriate to other departments/community setting.

Direct patient and carer contact.
Follow up patient enquiries on a day-to-day basis liaising with the acute oncology nurse consultant and initiate appropriate response according to protocols and individual pathways, liaising with the team as appropriate when non routine.
Complete Holistic Needs Assessment with patients utilising good communication skills, ensuring pertinent information is shared with the key worker within an acceptable timeframe.
Assists with the analysis of daily blood results and flags to nurse consultant or the relevant senior nurse specialist. Assists with tracking of relevant diagnostics test results and flags to nurse consultant or the relevant senior nurse specialist.. Liaises with appropriate departments for patient care/treatment.
Make pre planned telephone calls to patients to assess needs, liaise with the health care team as appropriate for proactive prevention approach to patient care.
Identify indicators of patient need or change in condition through telephone contact and respond appropriately as per protocol and /or liaison with team.
