Violet Home Care Ltd is a fast-growing Domiciliary Care Provider based in Epsom and Ewell. The company provide high standards of person-centred care across Epsom, Ewell and surrounding areas within Surrey.

The care coordinator is responsible for the continuity of care for all our clients. Being a Care Co-ordinator is about more than just rota management. You will be responsible for supporting the Registered Manger with Business Growth and ensuring we are always delivering the highest possible standard of care. This vital role has a huge impact on the lives of many people who rely on our care and support.

We pride ourselves on delivering care that truly meets the needs of the individual we support, and your role will be fundamental to ensuring this happens every day by matching clients and carers, not only based on availability, but also personality.

The ability to problem solve and think clearly when under pressure is essential for this important role in our company.

You will use your excellent:
- planning, communication, and organisational skills
- Ability to display empathy and warmth
- Ability to cope with pressure, whilst having a sound understanding of good care principles

To make sure that each client receives the care and support that they expect on time. This involves:

- Allocating care assistants with the right skills and experience to deliver the best quality care and support to each customer
- Organising care assistant rotas to minimise changes to the service and travel between customer’s homes
- Responding efficiently to day-to-day changes in the care and support packages
- Carrying out quality audits in line with CQC guidelines
- Carry out investigations relating to the quality of the service and implement improvement actions
- Manage the workflow and performance of the carers
- Daily rota management
- Contacting GP on behalf of clients.
- Ordering Medication for clients
- Contacting Social Services regarding client concerns.

The role offers the opportunity for developing positive working relationships with clients, their families, and the Care team. You will provide a crucial link between each client and their care assistant.

You will often deal with queries from customers about their care and support. This requires a sensitive understanding of customer needs and a supportive telephone manner.

The care coordinator must also be clear about the best way to work with care assistant schedules to maintain continuity in the delivery of care. You will need to build strong relationships with the care team so you can call upon extra cover when needed.

Care assistants may work over the weekend or evenings so the post holder must be willing to be part of the on-call emergency team rota which is currently 1:5

Full-time, Permanent

**Salary**: £18,198.00-£24,475.00 per year
