We are looking for a highly motivated Band 3 Therapy Support Worker to join our Stroke, Rehabilitation and Early Supported Discharge Team at the Shrewsbury and Telford Hospital NHS Trust.

We are looking for someone who has previous Band 2 or above Therapy Support Worker experience in either physiotherapy and/ or occupational therapy teams. The post holder needs to be flexible, have good time management skills, IT skills, and work well in a team as well as independently.

The post is within the Early Supported Discharge Team, for 22.5hours a week. The working pattern can be discussed at interview.

You will be able to carry your own caseload completing assessments of patient's function, deliver rehabilitation and participate in discharge planning with guidance from qualified Therapists.

Therapy will be delivered in the patient's home setting either alone or with a colleague if required. There will also be administrative tasks to perform, such as (but not exclusive to) patient phone calls, equipment ordering and deliver/fitting, filing, car booking and diary planning to facilitate the team.

The Higher Therapy Support workers and Assistant Practitioners in our Inpatient and ESD team work between the physiotherapy, occupational therapy and speech and language teams. If required, the successful post holder will be offered training to develop speech and language therapy competencies.

We are delighted to be able to offer patients who have had a stroke the opportunity of assessment and treatment over the weekend and Bank Holidays and this post holder would be expected to participate in the 7-day working rota for this. This is currently within the Stroke inpatient wards at Princess Royal Hospital Telford.

The Stroke Early Supported Discharge team is based in Shrewsbury and the acute stroke service is based at The Princess Royal Hospital, Telford. The team is line managed by the same therapy team managers.
