We have an exciting opportunity for you to join an established innovative Stroke team at Lancashire Teaching Hospitals NHS Foundation Trust. You will join our existing Stroke Consultants and Nurse Consultants to help develop our stroke services as we move to comprehensive stroke centre status and you will play an instrumental part in the development of our ambulatory stroke pathway in line with the regional integrated care service (ICS).

You will be committed to providing excellent direct independent clinical practice, be a team player, and passionate about stroke.

In Advanced Practice on Stroke we are committed to building on your existing skills and knowledge and welcome the opportunity to develop you. Full support will be given to your personal and professional development through regular appraisals; in house training sessions and study time.

The Division of Medicine is looking to add to our team of enthusiastic, highly skilled and motivated Advanced Specialist Practitioners (ASP’s). We are keen to hear from both fully qualified and experienced ASP’s, or from those in senior positions with significant medical experience who would wish to train to become an ASP in our Stroke service.

You would be responsible for ensuring the assessing, planning, management and evaluation of care to meet the personalised health and well-being care needs of patients, in line with the Trust’s Values & Professional Frameworks Provide strong leadership and direction to the team, monitoring and maintaining standards of care and professional practice to support day to day efficient running of the clinical area, service, in line with the 4 pillars of Advanced Practice.

To practice autonomously with a high level of accountability at an advanced clinical level in the direct delivery of high quality care, making critical clinical decisions based on advanced clinical skills assessment.

Plan and manage complete episodes of care, working in partnership with others and delegating and referring as appropriate to optimise health outcomes and resource use, as well as providing direct support to patients and clients

To provide clear leadership and direction to support both the medical and nursing teams acting as a role model for the Trust, presenting a positive image to patients and staff

Advocate and contribute to the development of an organisational culture that supports continuous learning and development, evidence-based practice and succession planning

Critically engage in research activity, adhering togood research practice guidance, so that evidence basedstrategies are developed and applied to enhance quality, safety, productivity and value for money

As an innovator, role model and clinical leader embrace and implement the vision and values of LTHTR and the stroke service
