To maintain effective communication with the teams and the children and families being supported and keep a detailed documentation of care as directed by registered staff, to record accurately within the patient record including home documentation and Rio systems. Provide and receive complex and sensitive information whilst supporting difficult conversations. Communicating with parents and reinforcing information provided particularly where there are barriers to understanding such as learning disabilities or language ensuring there is equity and diversity in all care provision and unique and individual needs of all parents, carers and families are met with kindness, diversity and dignity. To support others and self in managing the emotional elements of this role, attending team debrief/reflections accessing counselling/Chaplaincy services as required.

Demonstrate sensitivity to the child and familys needs through maintenance of professional boundaries and respecting individuals privacy and dignity. Taking into account familys religious, spiritual and cultural beliefs in a respectful way. Undertake appropriate action if concerns arise in relation to child/young persons wellbeing i.e. safeguarding - reporting to registered nurse/Service Clinical Manager and other relevant professionals, accurately recording issues and actions.

To work with mínimal supervision using own initiative, planning own workload. Participate in team and MDT meetings to ensure effective communication is maintained in the best interests of the child/young person and family. To liaise with professionals within MDT to raise awareness of the team and what they are able to support with. Ensure stock of play and bereavement resources are replenished and stored safely.

On occasion support the teams and services out of hours when there are specific needs and requirements (such as focus groups, bereavement support and specific occasions such as memorial services and occasions of remembrance). Complete DATIX online forms accurately and when necessary. On occasion to support with the ordering of essential supplies and equipment. Understand accountability and the limitations of own role.

Under supervision of registered staff develop resources that will support the palliative care team with memory making. To participate in research relevant to role. Adhere to the Trust lone working policy and travel to visit clients across Birmingham City boundaries.
