Learning Disabilities and Transitions Specialist

An exciting opportunity has arisen for an experienced practitioner to develop and embed a new role in the organisation as the Learning Disabilities & Transitions Specialist Practitioner. This is a pivotal role in providing expert care for patients, families and their carers to ensure a positive experience of our services with excellent outcomes. We are looking for an experienced and enthusiastic individual who can develop our strategy to support the delivery of personalised care that is in line with best practice and national and regulatory standards.
We are seeking an individual who has the right values and behaviours to ensure we provide the best possible service for patients and families and carers.
The post holder will work collaboratively with key stakeholders to ensure that all users have a positive experience of the service, developing pathways of care that reflect the needs of patients, carers and families.
- To provide leadership, be a role model and demonstrate expert knowledge and high standards of clinical practice
- Directly influence and improve the quality of care received by individuals with a learning disability and/or autism, and facilitate open and easy access to trust services including when transitioning.
- Ensure smooth transfer of care and where an admission is clinically unavoidable, share appropriate information which will help to personalise the care the patient receives in hospital, and through supporting practice reduce length of stay where clinically safe and relevant to do so
- To promote patient autonomy through education and support so that individuals are able to make informed choices about their own care, now and in the future.
- To assist Trust staff to exercise their duty to provide people with a learning disability and/or autism, equality of access to the same healthcare as the general population, with added support to achieve the same health outcomes when transitioning across services, as those for the general population
- Develop partnership links and support the interface between the Trust and specialist services in order to help facilitate a smooth pathway when transitioning through services.
