**Key Duties and Responsibilities**

1.Identify and build relationships with people who are rough sleeping in Leicester, using inter-agency outreach sessions and regular visits to services/venues visited by service users.

2. Carry out Needs & Risk Assessments with identified rough sleepers to secure access to services which are suitable for their immediate needs. To work in partnership with the service user and any significant others involved in their support, to ensure assessed needs are translated into action plans and agreed goals. All support plans will be service user-led and reviewed regularly to ensure they are relevant to the client’s changing needs.

3.Identify barriers which make it difficult for individuals to access services and find solutions to overcome them.

4.Make appropriate referrals in partnership with rough sleepers to establish a sustainable life away from the streets as quickly as possible.

5. Assisting and empowering clients to develop life skills and participate in activities to promote increased independence and involvement with the community.

6. Participate in information and guidance service user ‘drop-in/hub’ sessions.

7. Producing, recording and maintaining accurate and useful information on clients and contact with them in order to report and promote effective service delivery and evaluation.

8.Work in close partnership with LCC, transitions team and other homelessness services.

9.Respond to reports of cases of rough sleeping in Leicester and attend regular hotspots.

10.Offer support and advice to enable reconnections where relevant.

11.Give service users information, sign posting and provide and support and advocacy so that they can access appropriate services as easily as possible.

12.Provide floating support to aid the progression for a targeted group of individuals.

13.Liaise with relevant agencies and, with the agreement of the service user, act as an advocate with those agencies on the service user’s behalf.

14.Maintain records in line with the service operational procedures, including creating and maintaining accurate case notes and action plans.

15.To assist service users to integrate with relevant local services including housing, health, social care, substance use and social opportunities.

16.Adhere to the organisation’s working protocols with relevant agencies that allow information sharing and joint working to benefit the service user.

17.Provide monitoring information as required by the service.

18.Positively promote the service to external agencies and the relevant statutory bodies.

19.To minimise risks to rough sleepers by identifying, reporting and following up any safeguarding concerns and incidents.

20.To work flexibly with rough sleepers to meet emerging support needs, identifying longer term support needs and well as immediate support needs.

21.To coordinate/arrange cover for outreach when there is annual leave and/or sickness.

3.To perform as part of the wider HTH team, attending and participating in regular meetings, appropriate training courses and in skills sharing sessions.
