**RESPONSIBLE FOR**:
The Homeless Engagement Practitioner (HEP) role exists to provide a bridge between primary and secondary care, supporting homeless patients who are in the hospital, working with the hospital and the patient for a successful discharge, and reducing unnecessary hospital attendance and bed days.

**JOB PURPOSE**:
**In-reach into the hospital and facilitate discharge**
Homeless people can often have prolonged hospital stays and unsatisfactory discharge without appropriate support. They can also struggle to maintain a hospital stay when necessary. Leicester City Council’s Rough
Sleeper Initiative and Changing Futures work provide a multi-agency collaborative approach to supporting those who are homeless and/or rough sleeping.

By maintaining contacts within the wider community homeless health and social care multidisciplinary team, the Homeless Engagement Practitioner supports homeless people who are inpatients and liaises with the ward teams to ensure a smooth journey through treatment and a timely discharge, with robust follow
- up in primary care.

Work closely with Local Authority accommodation providers to support service users in appropriate
Accommodation gaining knowledge and building pathways from health to accommodation.

Knowledge of the social support available **,** community nursing, and primary care support ensure the engagement practitioner can help the patient and UHL staff navigate the complex homeless system and facilitate the successful completion of the treatment episode and prompt effective discharge.

**Outreach into the community**
Working with the Multiagency RSI team and visiting patients in the community to help them navigate the healthcare system, including encouraging ongoing engagement with primary and secondary care in time management. Seeing the patient in their own environment and identifying medical needs plus helping with signposting to and liaising with community services who can help them.

**Reducing Inappropriate ED Attendances**:
The Homeless Engagement Practitioner will build close links with UHL’s frequent attendee nurse at ED to:
1. Identify frequent attenders in the department.

2. Work with the patient and the multi-disciplinary team to increase engagement in primary care.

3. Create and implement a structured care plan to reduce future ED attendances and admissions.

Through remote working the Homeless Engagement Practitioner also reviews the letters from ED and out
- of-hours services thus tracking patients’ use of these services and acting on these by arranging appropriate follow-up and management in primary care including emergency symptom management and help-seeking plans to proactively manage a worsening condition without needing hospital care.

**CLINICAL DUTIES**:

- Provide a holistic assessment including lifestyle advice/guidance/interventions on Health, diet, smoking,
