To support the Healthy Family Team in managing incoming and outgoing information via the telephone system and utilising SystmOne for recording information.

The post holder will perform all functions and tasks as required for the advice line team.

The Practitioner will take responsibility for the daily information flow via the telephone, including RECAP with liaison with B2 practitioner.

The post holder is responsible for the smooth transition of information from families and others to the Healthy Family Team.

The team will be working proactively using a partnership approach to promote health and contribute to tackling health inequalities within the level of competencies required for the post.

A key aspect of the role is to work flexibly to meet the needs of the service as required.

Work within the advice line team to support the Healthy Family Teams to deliver the 0-19 Healthy Child and Public Health Nursing Programme.

Work in partnership with other members of staff, teams, voluntary and statutory organisations in response to service user and local community needs contributing to the Public Health Priorities.

Signpost and provide information for parents and young people, communicating information in an understandable form to children and young people.

Undertake supervision from Professional Service Lead as per Trust policy.

Work with the Healthy Family Team to aid operational performance, informing Professional Lead of any noted anomalies.
