Job summary

To deliver the universal and targeted Healthy Child and Public Health Nursing Programme as part of the integrated Healthy Families Team. Working across the 0-19 age range under the direction of the Specialist Public Health Practitioner to improve the health and wellbeing of a defined geographical population

Main duties of the job

The post holder as a member of the Healthy Families Team will support the Specialist Public Health Practitioner in the delivery of a universal and targeted 0-19 Healthy Child and Public Health Nursing Programme. This will be delivered in line with the National and Local guidelines and public health priorities. Working proactively using a partnership approach to promote health and contribute to tackling health inequalities within the level of competencies required for the post.

The post holder will work with children, young people and families within the home environment, school or other identified community setting.

Job description

Job responsibilities

To undertake universal Healthy Child Programme checkpoint reviews, Tier One targeted interventions and public health interventions either 1:1 or groups, delegated by the Specialist Public Health Practitioner. Work as part of the Healthy Families Team to deliver 0-19 the Healthy Child and Public Health Nursing support the Specialist Public Health Practitioner in identifying level of need and delivery of targeted Tier One interventions eg. Sleep, behaviour, toilet training, healthy eating, emotional health and wellbeing. Work in partnership with other members of staff and teams, voluntary and statutory organisations in response to service user needs.
