Adult Social Care Single Point of Access team are looking to recruit a Social Care Assessor. This role supports the first point of contact for service users, carers and organisations wishing to refer into Adult Social Care for advice, information and community assessment.

Working alongside Social Workers and Occupational therapists you will be utilising a strength based approach and the three conversation model to promote independence and well-being. You will work in line with the Care Act to promote service users choice and control in their daily lives while respecting the key elements of the act to ensure service users can contribute to society and continue to have meaningful, autonomous and self-determined lives.

The role is varied and includes both working on the main telephone line giving advice and information as well as completing assessments in the community for care and support, which will also involve the provision and prescription of specialist adaptive disability equipment.

You should have some experience and knowledge of working in a health or social care setting and you must be able to communicate clearly and effectively both orally and in writing. You will have the ability to gather and record information in order to prioritise queries, resolve problems and signpost to alternative services. Working as a team member you will have the skills to manage potentially challenging and emotional situations, whilst treating people with dignity and respect.

The service is in the process of working towards integration with our health colleagues and in addition to this we are adopting new ways of working to improve services and the customer experience. The successful applicant will need to have a good understanding of the challenges we face and how the team can be supported going forward.

The post offers a good level of support and training, it offers an opportunity to expand knowledge and learn new skills within the social care arena. Adult Social Care offers comprehensive learning/access to training, a supportive management team, regular staff supervision and a personal development plan.

You must be able to travel around the borough independently to access service users’ homes and other office premises. Access to a vehicle and clean driving licence is required.
