**Job Title**: Social Care Assessor

**Location**: Enfield, London

**Salary**: £34,341 per annum

**Contract**: Full-time, Permanent

**Hours**: 37.5 per week

**Key Responsibilities**:

- Collaborate with service users to assess their needs and develop personalised care plans.
- Implement and support the execution of care plans to maximise individual service user development and opportunities.
- Establish and maintain effective working relationships with service users, demonstrating initiative in strengthening connections and fostering trust.
- Provide practical assistance and support to meet the needs of service users and their caregivers, ensuring their requirements are addressed comprehensively.
- Screen referrals, direct calls to relevant staff or teams, and maintain data spreadsheets to manage workflow efficiently.
- Assist in maintaining accurate records and documentation.
- Act as a key/case worker as required, conducting Care Act Reviews and ensuring the ongoing effectiveness of care plans.
- Ensure that service users have access to advocacy and support them in self-advocacy whenever appropriate.
- Promote the inclusion and participation of service users through various interventions such as group work and user participation initiatives.
- Undertake relevant training, contribute to team workshops, and engage in continuous learning opportunities to promote best practices.
- Work collaboratively with other professionals, carers, and agencies involved in supporting service users and their families.
- Adhere to statutory requirements, legislation, and council policies related to service delivery, data management, confidentiality, and financial regulations.
- Maintain accurate and up-to-date information using electronic recording systems.
- Assist in the induction of staff, provide on-the-job training to colleagues and students as necessary.

**Qualifications and Experience**:

- Experience in a health or social care setting, particularly with adults with diverse needs.
- Basic knowledge of relevant legislation, policies, and guidance.
- Familiarity with resources to support service provision and promote independence.
- Experience working within a Duty System.
- NVQ/QCF Level 3 or above in Health and Social Care (desirable).
- Minimum of two years’ experience in a Social Care Setting.
- Ability to travel independently within the borough, access service users’ homes, and office premises.
- Access to a vehicle and a clean driving license (preferable).
