**Role: Care Coordinator**

**Location: Bracknell**

**Salary: To £26,000 p.a. plus Bonus**

Working with a well-established specialist, bespoke and high quality care provider we are looking to recruit a Care Coordinator to join their rapidly growing service. In this role, you will be responsible for managing and overseeing clients and their support teams. You will liaise with case managers, clinical leads and families to ensure a bespoke care package is delivered efficiently and effectively.

You will have an understanding of the healthcare sector, excellent customer service skills and be highly organised.

**Key responsibilities include**:

- Providing exemplary customer service to all clients and carers by building and maintaining strong relationships
- Maintaining excellent communications with the Care Management team to ensure that the person-centred care is delivered at all times
- Improving and maintaining the continuity of care for new and existing care packages.
- Promptly allocate and confirm client bookings based on the clients' preferences and care skills required
- Maintain clear, accurate, appropriate and up-to-date digital records for all Clients and Care Workers within your area.
- Responsible for cordially resolving customer queries in a timely manner within your area to maintain a positive Company reputation.
- Provide regular weekly telephone support calls to all working carers and report any relevant feedback regarding carers/clients as required

**The successful applicant must possess**:

- Excellent communication and problem solving skills
- The ability to work under pressure and multi-task
- Strong IT skills and ability to use Microsoft Office
- Be highly organised and able to prioritise tasks with a flexible approach
- Able to maintain confidentiality and establish good working relationships

**Job Types**: Full-time, Permanent

**Salary**: £24,000.00-£26,000.00 per year

Schedule:

- Monday to Friday

Supplemental pay types:

- Quarterly bonus
