**Job Summary**
- disciplinary team but could also involve some remote working from home.

This role gives the exciting opportunity to work with a committed team and to help shape the delivery of services and projects to the patient population. This role will be pivotal to supporting the patient population of all the member Practices and will be specialising in remote data gathering and management.

**Overview of Braccan Health Network**

Braccan Health Network is an exciting new PCN which will be operating in Bracknell from 1st April 2021 serving a diverse population of 30,000 patients.

**Purpose of the Role**

Purpose of the role will be to support the Practices and Patients to improve screening. This role will be mainly based around collating data and contacting patients via iPlato, Simil and Accurix. This will be an evolving role as the Primary Care Network develops to the needs and health management of the population

**General Duties/ Key Responsibilities**:

- Proactively identify and work with a cohort of people to support their personalised care requirements, using the available decision support aids.
- Bring together all of a persons identified care and support needs, and explore their options to meet these into a single personalised care and support plan (PCSP), in line with best practice.
- Help people to manage their needs, answering their queries and supporting them to make appointments.
- Raise awareness of shared decision making and decision support tools, and assist people to be more prepared to have a shared decision making conversation.
- Ensure that people have good quality information to help them make choices about their care.
- Support people to understand their level of knowledge, skills and confidence (their Activation level) when engaging with their health and wellbeing.
- Assist people to access self-management education courses, peer support or interventions that support them in their health and wellbeing.
- Provide coordination and navigation for people and their carers across health and care services, alongside working closely with social prescribing link workers, health and wellbeing coaches and other primary care roles.
- Support the coordination and delivery of MDTs within PCNs.
- Production of relevant patient communications to improve performance such as mail merge letters, accurx, docmail.
- Creation of searches and/or protocols to help support performance improvement or monitoring within EMIS.
- Undertaking a variety of administrative duties to assist in the smooth running of the practice.
- Facilitating effective communication between patients, members of the primary health care team, secondary care and other associated healthcare agencies.
- Manage wait lists for Joint injections, and coils. Have had to create a wait list or thandi due to elevated demands for MH reviews.
- Care home management: assist with patient appointments, medication issuing issues, proxy access, annual reviews and liaison between care home and the surgery.
- LTC recalls (Long Term conditions - Annual Reviews).
- Registration of new patients.

**Job Types**: Full-time, Permanent

Pay: £12.20 per hour
