Discharge Coordinator

To facilitate and ensure there is a discharge plan for all patients on the designated Wards, taking steps to avoid delays;
- Actively communicate with patients and their carers with the continuous process of discharge planning/transfers of care and ensure they are involved at every stage;
- Be responsible for the co-ordination of patient discharge/transfer to different centres and to support the development, implementation and evaluation of such processes;
- Liaise with Registered Nurses, Doctors, Therapy Services, Social Services, CHC team, Community teams and any other appropriate members of the Multidisciplinary team to provide a proactive and continuous process of discharge planning.
- To actively assist the Wards, working alongside the Nursing Team, Occupational Therapy and Physiotherapy Service in the development and delivery of effective patient rehabilitation and health promotion programmes. To work autonomously in independently carrying out triage, assessment, treatment planning, modification, follow up and discharge for a designated caseload, working in liaison with qualified staff.

1. To carry out the appropriate procedures and related tasks that have been selected and delegated by the registered nurse or therapist with the appropriate level of competency. It is the joint responsibility of the discharge assistant and the supervisor to ensure the assistant is competent and confident in carrying out the required intervention and duties. These may involve mobilising and transferring patients, assisting with functional tasks, assisting with personal care tasks, supervising exercise programmes and liaising with other members of the MDT or Centres in order to facilitate discharge.
1. To help support on a daily basis discharges from the ward areas to a variety of destinations i.e. discharge lounge, home, back to existing support services, other hospitals; in keeping with the needs and wishes of patients and their families, acting as a patients advocate when necessary.
