HSIOWICB is responsible for planning and funding local health services across Hampshire. We are looking for an Assistant Administrator to join the Continuing Healthcare team covering the Hampshire area. This is a key role within a strong and supportive team and is an exciting opportunity for an individual to play a key part in successfully delivering important services for the Hampshire. (West Hampshire, North Hampshire, North East Hampshire, South East Hampshire, Fareham & Gosport).

The duties and responsibilities that you will be expected to undertake may include:
Checking and maintaining written and electronic records, databases.

Some general office duties on rotation to manage outgoing post.

Administrator experience is required, it is essential that you are educated to NVQ Level 3 or equivalent qualification (or experience level), together with excellent communication skills.

You should be self-motivated, highly organised, flexible and have a can-do attitude.

Hours of work are 37.5 hours per week between Monday to Friday 9-5. Although our base is Fareham Civic Offices, we currently work from our homes. Expectation to attend the office for mandatory training, post rotation and any other business.

You should note that we generally appoint at the first pay point of the scale indicated in the advert, unless you are transferring from another NHS organisation when other arrangements apply.
- To work alongside colleagues from adult social care and provider health services to support the discharge to assess process by ensuring that patients are discharged from acute hospital in a timely way and are assessed for eligibility for CHC in community settings where possible.
- To undertake quality assessments and safeguarding reviews within all settings where CCG funded patients are cared for to ensure that individuals care needs are met.
- Through the CHC duty system, respond promptly and efficiently to a variety of tasks focusing on discharge and care planning from hospital. Confidently and professionally respond to crisis within existing packages of care funded through Continuing Healthcare to ensure individuals care needs are met.

- To coordinate and participate in Continuing Healthcare assessments, and subsequent monitoring and reviews for individuals deemed as eligible for Continuing Healthcare in an appropriate setting for their care needs, which may be in their own home, rest or nursing homes. This will include quality reviews of packages of care and placements of clients funded through Continuing Healthcare.
- To case manage a small number of named individuals with complex needs living in the community who are deemed as eligible for Continuing Healthcare.
- To work with Children’s services to ensure a smooth transition into Adult Continuing Healthcare for those young adults who are deemed eligible for Continuing Healthcare.
