**Advanced Practitioner -Community Team - Merseyside**

**£37 per hour umbrella**

**About the job**:
Are you an experienced and dedicated Social Work Practitioner seeking a challenging role in a dynamic community-focused environment? 3D Recruit is currently recruiting for an Advanced Social Work Practitioner to join a Community Team. This is a unique opportunity to play a key role in shaping and delivering high-quality social care services to individuals and families within our community. It is a locum role where you will be on an initial 3-month contract with the potential to be extended

**As an Advanced Practitioner within this team, you will have the following responsibilities**:

- **Assessment and Intervention**: Conduct in-depth assessments of individuals and families within the community, identifying needs and risks. Develop and implement targeted intervention plans to address complex social issues and promote positive outcomes.
- **Supervision and Mentorship**: Provide supervision and mentorship to less experienced team members. Foster a culture of continuous learning and development within the team, promoting best practice and maintaining high standards of professional conduct.
- **Collaboration and Partnership**: Work collaboratively with local agencies, community organizations, and healthcare providers to create a network of support for individuals and families. Build strong partnerships to enhance the overall well-being of the community.
- **Complex Case Management**: Take the lead on managing complex cases, providing expertise and guidance to colleagues. Ensure that all relevant legal and procedural requirements are met while delivering person-centred, holistic support.
- **Service Development**: Contribute to the ongoing development and improvement of social care services within the community. Identify areas for enhancement, implement innovative solutions, and participate in strategic planning to meet the evolving needs of the community.

**Requirements**:

- 3 years of qualified Social Work experience
- 5 years of satisfactory references
- Driving License with access to a vehicle
- SWE Registered
