The vision for an integrated discharge team in Bolton is for the three main stakeholders, the Trust, the Council and the CCG to work in partnership to provide a fully integrated and co-ordinated hospital discharge service for patients admitted to complex care wards in Royal Bolton Hospital

As a Navigator you will promote and facilitate timely, safe and effective discharge arrangements as an integral part of care management throughout the Trust in close partnership with Trust staff, external agencies and other provider organisations.

To provide discharge support and advice to the integrated discharge team. You will be required to attend MDT meetings on your nominated group of wards across the Trust, and identify patients suitable for one of the discharge pathways. This will be a 7 day discharge service, operating 8am - 6pm.

To promote and facilitate timely, safe and effective discharge arrangements as an integral part of care management throughout the Trust in close partnership with Trust staff, external agencies and other provider organisations.

To ensure full compliance by both the hospital and patient with all requirements to enable effective and efficient admission, treatment and discharge.

To actively participate in development of the Trusts discharge planning arrangements.

The Integrated Discharge Team is consists of NHS and Council members of staff working together to facilitate safe discharge for patients in line with the national hospital discharge policy. You will be based in one of three hubs, working alongside an integrated team of nurses, navigators, community assessment offices, and social workers.

You are encouraged to maintain your Covid vaccination status unless medically exempt and will be supported to access information and guidance, to inform decisions that supports vaccine confidence, protects yourself and patients.
