CTD Care Services Ltd offers a comprehensive range of personal and practical care services. The services are tailored to meet individual needs and can be provided on a short-term or long-term basis. CTD Care Services Ltd provide support and advice to our individuals and their representatives and will ensure that the care is delivered in a safe and respectful manner.

**Duties**:

- Coordinate and manage patient/individuals care activities
- Manage the staff rota and allocation systems to ensure calls are fully staffed and attended
- Schedule appointments and follow-up visits
- Communicate with healthcare providers and patients to ensure continuity of care
- Assist in the development and implementation of care plans
- Maintain accurate and up-to-date patient/individual records
- Provide support and education to patients/individuals and their families
- Collaborate with multi-disciplinary teams to optimize care outcomes
- Monitor patient/individuals progress and make appropriate adjustments to care plans as needed

**Experience**:

- Previous experience in a healthcare office setting required
- Knowledge of social care terminology and procedures
- Proficient in using office software, such as Microsoft Office Suite
- Excellent communication and interpersonal skills
- Strong organizational and time management abilities
- Ability to work independently and as part of a team

**Salary**: £23,271.00-£26,452.00 per year

Expected hours: 30 - 37.5 per week
