We are looking for someone who has a track record of being innovative and who really wants to challenge how services are provided for children or young people with a learning disability or autism when it comes to hospital care in the Thames Valley.
Come and join our small but rapidly growing team to help set up the service and shape not only your own role, but the role of the team. A new and exciting opportunity has become available for a Modern Matron in the Learning Disability and Autism (LDA) Hospital at Home service being developed by the Thames Valley CAMHS Tier 4 Provider Collaborative.
In this development opportunity, you will be coached by senior clinicians and managers for 12 months, while you build up your skills and knowledge of moderate to severe Learning Disability and Autism and help create a pioneering service.
LDA Hospital at Home is a specialist service working with patients up to 18 years old with moderate to severe learning disability and autism. The Thames Valley Provider Collaborative is a network of service providers covering Oxfordshire, Buckinghamshire, Berkshire, Gloucestershire, Swindon, Wiltshire and Bath and North East Somerset (BaNES), working collaboratively to provide high quality care for the most unwell young people as close to home as possible.
We are currently recruiting our multi-disciplinary team (MDT) who will work with young people diagnosed with moderate to severe learning disabilities and autism, who are at risk of admission due to mental health difficulties. The post holder will also work with current inpatients with moderate to severe learning disabilities and autism to ensure they have as short a stay in hospital as possible and facilitate timely discharge and transition home. The service will work to ensure suitable packages of care are in place for young people at risk of admission.
The LDA Hospital at Home approach is based on the Tier 4 ‘assertive outreach’ model, offering an enhanced package of support to young people receiving care from the Mental Health, Learning Disability and Autism Community Services and Keyworker services and identified as at risk of admission on the Dynamic Risk Support Register. Care will be delivered in the community and virtually in a blended therapeutic model.
