We are looking for a dynamic, enthusiastic, positive forward thinking and organised individual to support the corporate cancer strategy team in the development and improvement of services we offer to our patients here at the cancer centre at Lancashire Teaching Hospitals.

You would be working in a friendly and supportive team. The role of the cancer service improvement facilitator focuses on supporting the clinical teams in cancer with quality improvement projects and support the delivery of the cancer transformation programme. This is an exciting opportunity to be involved in: improving the quality of care, transformation of pathways, improving experience for our cancer patients and their carers. - Work with key people across the health economy to jointly establish the aims, targets and scope for improvement projects, subsequently co-ordinating or participating in any cancer projects the trust is involved with according to the agreed aims
- To work effectively with the trusts cancer strategy team in the delivery of the cancer transformation programme
- To ensure the trust meets the national cancer targets by ensuring best practice cancer pathways are in place for all tumour sites
- Assist clinical teams in cancer and end of life with setting up systems for measuring improvement and to co-ordinate the analysis and present data in a variety of formats
- To ensure all teams adhered to the national annual cancer peer review programme
- Along with the lead cancer nurse facilitate and oversee a portfolio of ongoing quality improvement projects. Including leading pathway development by identifying and promoting the need for change and motivating the clinical teams to continually develop improvements for the benefit of patients.
- Analyse and monitor complex information on multiple trust IT systems
- Develop links and liaise with clinical and managerial colleagues at national regional and local levels to encourage sharing of best practice to continually improve pathways
- Work with the clinical teams to develop and embed personalised care across whole pathways personalised stratified follow up for all cancer pathways

- To provide practical support to the cancer multi-disciplinary clinical teams at Lancashire Teaching Hospitals NHS Foundation Trust in taking forward any service improvement initiatives and encouraging new ways of working
- To provide support to the cancer MDTs in the continual redesign/improvement of cancer patient pathways as part of the cancer transformation programme
- To actively ensure the voice and needs of patients and carers are at the heart of all service improvement activity
- To ensure effective communication and engagement of all stakeholders
- Provide expert project management advice and support to other staff and teams across the trust and other organisations such as the cancer alliance
- Integrating the work of the national cancer delivery plan into every day working
- Proactively promote a positive image of service improvement and its benefits to patients
- Spread and implement the continuous improvement methodology across LTHTR using project management techniques
- Using continuous improvement methodology to improve organisation and flow of cancer pathways, and implement pathway action plans to deliver optimal care and experience for patients
