We are developing new keyworker roles in our Community Mental Health Teams. These roles involve working with people with mental health related needs in our community, and being a pivotal part of our new community mental health teams. Being a keyworker involves building positive relationships with the people on your caseload, developing co-produced care plans, being their main point of contact, and working with a range of mental health professionals and voluntary services to meet needs effectively. This is a truly rewarding role and will give you the opportunity to make a significant difference to those the team support.

We are looking for Keyworkers who are passionate about making a difference to the lives of people who have a variety of mental health conditions. You will form part of a multi-agency multidisciplinary team to include Nursing, Social Work, OT, support work, Psychology and Psychiatry and voluntary sector staff (This list is not exhaustive)

This is a truly exciting time in which the Community Mental Health Transformation Programme will change what we do and how we deliver community mental health services.

Based in Coventry
- You will work closely with clinical leads, managers and colleagues to develop the service and manage a caseload of people. You will help develop treatment pathways, processes and structures as well as building effective multiagency links to ensure that we meet the diverse needs of the people we see.

You will complete outcome measures to show progress made and identify areas of needs. You will ensure that the needs of the people you see are met and that will involve drawing in other staff in support of this.
