- £26,960 - £30,960 per annum (includes on call payment - 1 in 5)
- Monday - Friday, 9am - 5pm

The service we operate is specialist and Nurse Led, we provide highly trained care teams to work closely with clients in the community. We rely on Healthcare Assistants, Support Workers and Registered Nurses to support our service users within their own homes. Behind the scenes in the North West operation, we have a team of staff who work to co-ordinate and manage all aspects of our service.

**It is essential that you are a driver with access to a vehicle.**

**Main responsibilities as a Care Coordinator**:

- Building relationships with commissioners, professionals, clients and family members
- Rota Management for your packages of care
- Responding efficiently to day to day changes in your care packages
- Providing excellent customer service
