- Care Coordinator (Complex Care)
- Heywood, Lancashire
- Full Time, Permanent
- Monday - Friday, 9am - 5pm
- £28,060 - £32,060 per annum (includes on call payment - 1 in 5 rota basis)
- Opportunity to earn bonus of upto £2,000 per annum
Apollo Home Healthcare is a national provider of high-quality, complex homecare services for adults and children.
We operate a specialist, Nurse Led service and provide highly trained care teams to support our service users within their own homes, enabling them to live as as independently as possible.
Behind the scenes in our North West operation, we have a dedicated team who work to co-ordinate and manage all aspects of our service.
The role of Care Coordinator is fast-paced and varied and plays a key part in ensuring that our service users receive a high quality, complex care service that is centered around their specific needs.
You’ll oversee and manage a caseload of complex care packages and be the first point of contact for our service users, their families and other healthcare professionals, so it’s essential that you are a clear communicator who can build trusting relationships.
**In addition to the above you will also be responsible for**:
- Responding and managing day to day changes within your care packages
- Rota Management
- Management of Health Care Staff including reviews, appraisals and spot checks
- Scheduling monthly meetings with service users
- Care Plan reviews
**What we look for**:
- Previous care coordination experience (essential)
- Excellent customer services skills
- Strong organisational and admin skills
- Like-minded individuals, aligned with our values

- Drivers with access to a vehicle
