The Patient Safety Coordinator is responsible for the effective coordination and administration of the Trust's patient safety investigation processes, including the coordination of the rapid review process and ensuring that all reported incidents are appropriately triaged and managed within the team.

The Patient Safety Coordinator will support the Patient Safety Team ensuring that effective relationships between the Board of Directors, Directorate Senior Management Teams, and Investigators are maintained.The post holder is required to be able to negotiate and influence by fostering relationships both internally and externally.

The Patient Safety Coordinator will support the Patient Safety Facilitator in reviewing incident reports, and be able to process complex and sensitive information.

The Patient Safety Coordinator is responsible for providing administration for the trust's Rapid Review Group, and will require well-developed organisation and planning skills to ensure the effective running of the meeting.

The post holder is required to have excellent communication and analytical skills to deal with the day to day challenges of maintaining an efficient and effective service to the Patient Safety Team.

- Chase, prepare, collate and distribute the papers for the Rapid Review group.
- Ensure appropriate attendees are invited to Rapid Review to enable good governance processes regarding the ratification of Investigation Terms of References, and Serious Incident Investigation Reports, and compliance with the national timeframes.
- Ensure accurate records of the Rapid Review Group meetings are held and maintained.
- Attend and contribute to the Rapid Review Group meetings, producing and circulating minutes of the meeting and an action log.
- Exercise judgement when responding to complex queries from internal staff and external organisations in relation to incident investigations, Duty of Candour and internal and national guidelines relating to patient safety.
- Support the use of Structured Judgement Reviews as an investigative tool as part of the implementation of the Patient Safety Incident Response Framework (PSIRF).
- Review Datix Incident Reports and identify incidents for escalation to further investigation.
- Develop, establish and maintain efficient systems and processes to ensure good governance and enable the effective and accurate collection, collation and reporting of routine data and information, producing statistics when required.
