**Role Purpose**
- To work closely with the Rainbow Court office staff in supporting our customers to maintain an independent lifestyle, to widen the areas of experience, by enabling them to make meaningful choices, reducing isolation and improving access to information, communication and mobility and provide support in all aspects of daily living with high quality communication support service, putting the customer at the heart of what we do at all times.
**Key responsibilities**
- In accordance with the customer’s individual support plan deliver a support service through consultation and agreement with the customer. Assist the office to devise and implement, where appropriate, suitable support methods to ensure that identified support needs are met.
- To ensure you deliver the agreed support and communication support as described in the customer’s individual support plans.
- Understand and recognise that a customer may sometimes communicate what is important to them by his or her behaviour. Sometimes this can appear challenging, you must recognise this and follow the guidelines set out to enable us to respond in a consistent and appropriate way.
- To provide safe mobility support and the ability to adapt your working practice in line with the individual skills and needs of the customer.
- Provide communication, guiding and support to enable the customer to self-advocate and act independently in their affairs and within the local community. This may include enabling the individual to take on tasks like shopping, payment of bills, routine health appointments, and management of personal finances and other matters.
- Liaise as and when required with Social Workers, families and other circles of support, and always keep the office up to date up to date.
- Ensure all safeguarding practices are always adhered to and supported.
- Reinforce and promote the use of appropriate communication skills between the customer and external contacts.
- Through discussion and agreement with the customer and the office, listen and respond to the customer you are supporting.
- Support the office in the production of customer support plans, customer support reviews and risk assessments. Ensure you are compliant to General Data Protection Regulations and in accordance with DBUK policy.
- To support the office to ensure risk assessments are conducted for all customers, are complied with and updated considering changing support needs.
- Participate in case review meetings if requested.
- Support the customer with any personal care tasks, which may include, washing, bathing, shaving, dressing and using the toilet, managing or assisting with medication, moving/handling in line with Health & Safety, movement orientation and mobility, food preparation, assistance with eating. In these tasks you will ensure that a person’s right to dignity, privacy and cleanliness are respected.
- Promote the health and wellbeing of the customer by encouraging and supporting a healthy lifestyle, this could include diet, exercise and paying attention to medical needs, highlighting any issues of concern to the appropriate person.
- Recognise the need to provide the customer with relevant information about the environment around them.
- In line with the personal support plan, actively work with and encourage the customer in identifying and participating in a wide range of enjoyable, stimulating and purposeful activities.
- Where relevant ensure that the domestic setting is maintained to a high standard producing a safe environment. This may include support with household tasks enabling the individual to be as fully involved as possible in the running of their home to develop the person’s daily living skills.
- Support the customer where necessary to maintain contacts with family and friends and extend their range of social contacts and access to the local community.
- Liaise with the customer on issues related to housing, health, social care, benefits, welfare and advocacy as appropriate and in accordance with care plan and the office.
**Further responsibilities**
- To ensure compliance with DBUK policies and procedures and CQC Fundamental Standards, including those to protect the customer from abuse, this includes procedures on adult safeguarding and finances.
- To undertake mandatory and personal development training. To complete DBUK corporate induction process, also to participate in the initial induction to your role, which will include completion of the Care Certificate and any other specific training that may be relevant to your role.
- As specified by the office, to attend regular team meetings and personal 1:1’s and personal development reviews.
