Duties and Responsibilities Collaborate closely with healthcare professionals, including paediatricians, dietitians, psychologists, and physical activity specialists, to facilitate comprehensive care for CYP in the weight management program through Multi-Disciplinary Team meetings. Coordinate and manage patient care by providing personalised guidance, appointment scheduling, and effective communication to ensure CYP receive the necessary support. Assist CYP and their families in navigating the weight management program, offering guidance, educational resources, and empowering them to make informed decisions. Create and maintain personalized care plans for each CYP, leveraging specialized tools and techniques to tailor interventions and support.

Provide families with clear, accessible information to help them understand and actively participate in the weight management journey. Support CYP in accessing educational resources, group activities, and peer support opportunities that enhance their physical and emotional well-being. Collaborate with dietitians and physical activity specialists to design and coordinate meal plans, activity routines, and behaviour change strategies. Participate in multi-disciplinary team meetings, contributing insights, sharing best practices, and fostering a holistic approach to CYP care.

Maintain accurate and confidential records of CYP consultations, progress, and relevant interactions. Adhere to service procedures, policies, and guidelines to ensure safe and effective care delivery. Contribute to the development and implementation of quality improvement initiatives, enhancing the overall effectiveness of the weight management service. Engage in ongoing professional development, staying updated on the latest developments in paediatric weight management and care coordination.

Collaborate with external partners, community organizations, and schools to promote the program and facilitate referrals. Provide support in managing inquiries, concerns, and feedback from CYP and their families. Actively participate in team meetings, training sessions, and continuous learning opportunities. The postholder will be required, when and where appropriate to the role, to comply with the processing of requests under the Freedom of Information Act 2000 The postholder must comply with ABLs policies that protect the information assets of the organisation from unauthorised disclosure, modification, destruction, inappropriate access or use. The postholder will be responsible for maintaining the clinical and/or corporate records that fall within the remit of this role to the standards in ABLs records management policies, and data quality processes and standards. Health & Safety Compliance with the Health & Safety at Work Act 1974 the postholder is required to fulfil a proactive role towards the management of risk in all of their actions.

This entails the risk assessment of all situations, the taking of appropriate actions and reporting of all incidents, near misses and hazards, and a statutory duty of care for their own personal safety and that of others who may be affected by their acts or omissions.
