The package is for a 53 year-old female who requires 2:1 support during the day to access the community as well as night time support.

She lives in a supported living setting.

Her needs are complex - Diabetic. Paranoid schizophrenia. Psychotic symptoms. Auditory hallucinations. History of self-neglect. DVT.

: she struggles with her intake of sugary food and drink which can affect her diabetes and DVT. She’s at risk of sores on her feet and increase in weight. She also eats hot food too quickly which can cause choking and burns. Needs encouragement to eat a healthy diet and support with shopping choices. Reminders to let her food cool down and eat slowly. Reminders to continue with diabetic checks and hospital appointments.

: The clients’ family have a history of cardiomyopathy which is increased with the use of antipsychotic medications. This needs to be monitored every 3-6 months. Carers are required to monitor for potential side effects. The client does spit and hit out so she will require support time to discuss her mental health symptoms and mood. Encouragement for the continuation of her medication is needed and support provided with clear boundaries.

: The client does have history of overdoses in the past as well as self-harm. She will require support to discuss any concerns. She administers her medication herself so monitoring and support is required.

Self harm history which is largely impacted by her mental health. The client is at risk of DVT so the carer is required to look out for signs and symptoms.

: Staff are required to support with activities that have been planned. Support is also required of the needs to comply with the terms of her tenancy to maintain her supported housing.

Shifts will be 8am-8pm or 8pm-8am

**Job Types**: Part-time, Zero hours contract

**Salary**: £12.00-£36.00 per hour

Expected hours: 16 - 30 per week
