The Somerset Treatment Escalation plan (STEP) form is a key communication tool of the clinical discussions with a person to ensure their care is right should they not be able to speak for themselves. Behind the form is an individualised and caring discussion between person and professional.

**JOB SUMMARY**:
The post holder will be responsible across somerset to support our staff
- clinical practice concerns and questions around using the TEP accurately
- development and implementation of the TEP policy
- TEP education for colleagues working within acute and community care

The post holder will work closely with other professionals, patients and carers within the community to improve use of the TEP in terms of both quality and accessibility.

The post holder will review and seek to influence and improve TEP implementation across Somerset.

This role is part of the integrated approach to improve TEP engagement within Somerset NHS Foundation Trust and the wider Integrated Care System; the organisations aim to be an outstanding provider of supportive, palliative and end of life care.

Consistent and considered use of the TEP as part of a wide offer of advance care planning will facilitate this.

Identify training needs for TEP engagement and completion to required standard in the acute and community settings, and develop a programme of training collaborating with key professionals to co-ordinate

Promote and participate in joint working with other organisations involved in the development of TEP practice to coordinate, explore, encourage and support the sharing of good practice, experience and learning via local resource platforms

Work closely with health care providers in Somerset to deliver key factors of NICE guidance QS13, QS144 and NG142;
Display leadership skills and an awareness of effective change management in order to facilitate learning and raise awareness of national standards to empower all professionals to confidently advocate for patients and their families;

Attend, contribute to and participate at local and network-wide meetings and conferences as necessary;
