The Early Supported Stroke Discharge (ESSD) service is excited to advertise a post for 30 permanent hours for a stroke-specific Rehabilitation Assistant to work within the Carlisle localities. Our team is based at Hilltop Heights in Carlisle and you will be working with people in their own homes as well as across multiple NHS & community sites depending on the needs of the stroke survivor. The ESSD service plays a vital role in safe, supported & early discharge from the Hyper-acute Stroke Unit & the stroke ward at the Cumberland Infirmary & we often work with our stroke survivors before their discharge from hospital.
