**Are you an experienced care professional who is looking for a rewarding career?**

We are seeking an experienced Rehabilitation Support Worker to enable an adult to get out into the community and attend appointments as required.

**Your duties as a Rehabilitation Support Worker**:
Your duties will include providing them with assistance to get out into the community to do activities that they enjoy. Someone suitable for the role would be anyone interested in local history, motorbikes, films, wildlife.

You will be required to take the client out into the community so that they can complete their shopping duties and being socially involved with other people in the community.

You will also attend medical appointments with the client; other duties will include encouraging the client to engage in therapy programmes set out alongside other healthcare professionals to assist the client live independently.

This role of Rehabilitation Support Worker is a lone working role, and the client has no gender preference.

**Key skills, knowledge and experience**:

- You should have previous experience of working within the health and social care sector.
- Driving licence and car owner is essential.
- You should be trained to NVQ/Diploma Level 2 in Health and Social Care (or have equivalent experience e.g. completed year 1/level 4 of a recognised health profession degree course)

Our Client is specialist case management and rehabilitation company providing Support Work, Case Management, Occupational Therapy, Physiotherapy and Medico Legal services to children and adults across North East England, North West England and Scotland.
