Magnolia House Surgery has an expanding team who all work well together and offer support. This includes nursing staff, paramedics, clinical pharmacists, pharmacy technicians, care co-ordinators, Social prescribers and many more. We are a training practice with a range of specialist interests which we actively encourage and would welcome. Current interests within the surgery include minor surgery, joint injections, LARC fitting and a supportive and established member practice of the Ascot Primary Care Network and local federation Berkshire Primary Care.

Monday 8am - 6pm

Tuesday 8am - 6pm

Wednesday off

Thursday 8am - 6pm

Friday - 8:00am - 5:30

**JOB TITLE: Clinical Coordinator / GP Assistant**

**REPORTS TO: Practice Manager**

**1. Job Summary**

To be responsible for the leadership and supervision of a team of administrative staff, ensuring all administrative duties are performed effectively and to the required standard, meeting the objectives of the practice. Support the management team in promoting ED&I, SHEF, Quality & CI, Confidentiality, Collaborative Working, Service Delivery, Learning and Development and carry out other duties as directed by the management team.

The post-holder will streamline the flow of clinical correspondence through the organisation accurately and to summarise and read code relevant information from patient’s clinical records into the practice computer system, working strictly in accordance with the practice notes summarising protocol.

**2. Roles and Responsibilities**

The following are the core responsibilities of the Senior Clinical Coordinator. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels.

The Senior Clinical Coordinator is responsible for looking after there allocated clinincans and completing the following tasks:

- Drafting ongoing sicknotes
- Arranging follow up appointments/ follow up care for your patients.
- Referrals
- Helping with medicine management
- General IT issues
- Docman tasks
- Taking clinical meeting minutes
- Drafting safeguarding reports (17 & 47 requests) for you to approve to be sent.
- Processing to whom it may concern letters.
- Pre-fill any HGV/Taxi medical forms before the appointment.
- Processing ICE request for things like blood tests x-ray’s DEXA scans
- Help liaises with outside agencies.
- File all lateral flow results.
- Death admin

In addition, the post holder will identify any key clinical data from correspondence, such as new diagnoses, peak flow measures, BP recordings, height, weight, smoking status etc.

The role-holder will also assist in anything requested by the safeguarding lead with administrative duties as well as keeping safeguarding registers up to date. Act as super user for all DocMan and Emis workflow and act as a Smart Card RA if requested by the Practice Manager. The post holder will also summarise new patient medical records.

**2.1 Workflow Main Duties**
- Read code accurately diagnosis, procedures and investigations, identifying and carrying out actions e.g. contacting patients to make appointments or completing referrals
- Identify the complex or higher risk areas that require input or knowledge from the clinical team, including Safeguarding issues, working to the agreed Practice protocols
- Coordinating test requests i.e. bloods, x-rays, scans etc as required via the workflow process including contacting the patient, generating the test request form and arranging any follow up coordination as required
- Co-ordinating test requests with the Clinical Data Manager for QOF and recall purposes.
- Draft responses to consult letters for GPs

**2.2** **IT/EMIS/DOCMAN**
- Provide SMART card administrative support to the staff. Logging errors and ordering new and replacement cards when required if requested by the Practice Manager.
- Act as the practice super user for all DocMan related issues. Keep and manage an overview of Docman workflow, notifications, messages and any updates.
- Manage and maintain a regular overview of Emis workflow including tasks, lab reports, registrations, medicine management, GP2GP, document management, test requests, SCR and report management.
