The Prospering Place is a social care provider aiming to deliver high-quality person-centered specialist care in a semi-independent setting with tailor-made support packages for 16 to 21-year old's. The purpose of this role is to successfully support young people between the ages of 16-21 to transition into independent living whereby they feel confident and well equipped with the necessary skills to live a self-sufficient life. The post holder will join our friendly and professional team and will be required to provide support services in line with the Prospering Place guidelines. We have a home in Hounslow but the role may include floating support services in the surrounding areas.

**Key tasks and responsibilities**
- We expect all of our key workers to be passionate about making positive changes in young people’s lives, helping them achieve their goals and to aspire for positive outcomes. Providing support with all aspects of the young people’s daily lives and encouraging them to engage with positive activities.
- Support our young people gain vital skills such as cooking, cleaning and maintaining good personal hygiene, shopping, budgeting.
- Support young people with registering with a GP
- Build positive relationships with external agencies and stakeholders including social services, education providers, voluntary organization's, and to services that may provide support to young people such as drug and alcohol services, domestic violence support services, and voluntary organization's.
- Be able to lone work with young people and work autonomously as well as part of a team.
- Able to use initiative to visit young people who require floating support services
- Prepare individual key working reports regarding each young person’s progress; these reports must be completed to a high standard for submission to the placing authority and other relevant partner agencies.
- To manage and support an allocated caseload of young people aged 16+ by having key responsibility to ensure the provision and co-ordination of practical, personal, emotional, social and financial advice to enable a successful transition to adulthood.
- Assess the needs of young people by actively contributing to the assessment and preparation of the Pathway Plan to enable young people to achieve independent living.
- To oversee an allocated caseload of services and staff ensuring safe and high-quality service delivery in line with Ofsted National Minimum standards for supported accommodation.
- To ensure that services are prepared and ready for inspections. Ensuring that our organizational KPI’s are met monthly.
- Contribute to the involvement and participation of young people in the development and delivery of their Pathway Plans and develop creative approaches that will fully engage young people in the process i.e., seeking service user written feedback monthly and effectively implementing agreed changes and recommendations. Ensuring that young people are fully involved in and consulted in their care.
- Ensure that timescales to review Pathway and Care Plans are achieved and be responsible for convening additional reviews when circumstances significantly change
- To assist the organization with any Safeguarding or Disciplinary matters that may arise in the course of your duties. To promptly report any concerns to the appropriate Senior Manager

- Report any concerns regarding poor performance or conduct in a timely manner. Manage the duty phone on a rota basis and respond to emergency calls etc. out of hours when required which may include personally covering shifts.

**Skills required**

- Experience working in a semi independent setting or similar
- Be approachable and be a role model to young people
- Strong leadership skills
- Willingness to support young people to help them transition into adulthood by assisting them with cooking, cleaning, hygiene, budgeting, finding jobs and education etc.
