The post holder will work as part of a specialised multidisciplinary team providing respiratory care for a wide range of patients. Whilst the post holder will be based in secondary care, they will provide support to patients in their own homes. The post holder will assist the multidisciplinary team to improve patients’ quality of life by accepting delegated responsibility for monitoring progression of interventions.

To assist the team with support duties such as data collection and clerical assistance and stock control. To assist team in conducting outcome measures and/ or interventions on patients with Chronic Obstructive Pulmonary Disease (COPD) in adherence with departmental and Trust policies and protocols.

The duties and responsibilities for this post cannot be fully determined in advance and some flexibility will be required of the post holder. The post holder will be expected to take on additional duties following appropriate support and development.

The post holder will work as part of a specialised multidisciplinary team providing respiratory care for a wide range of patients. Whilst the post holder will be based in secondary care, they will provide support to patients in their own homes. The post holder will be responsible for providing both an efficient, comprehensive clerical/secretarial role and assist the team with the management of their patients. The duties and responsibilities for this post cannot be fully determined in advance and some flexibility will be required of the post holder. The post holder will be expected to take on additional duties following appropriate support and development.The post will involve many aspects of the Health Care Support worker and clerical workers roles. Primarily it will involve:
Caring for patients within the early supported discharge team.

Community visits of patients following an acute exacerbation of their respiratory condition.

Assisting in the assessing of patients for their suitability to the early supported discharge scheme.

The post holder would be required to work within a multidisciplinary team and must be aware of their personal and professional limitations. They will be required to work closely within approved protocols and guidelines.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
