A really exciting opportunity has arisen for individuals who are genuinely interested in supporting safe and effective patient discharge from the acute setting.

This is a new role which will involve working closely with ward managers and the ward multidisciplinary team, supporting all aspects of discharge planning at ward level.

The discharge assistant will be allocated to a specific ward on rotation and will develop close working relations with the ward staff and the Trusts discharge team. The post holder will also rotate into the Transfer of Care Hub - supporting all aspects of the discharge team’s work.

We believe that this is a genuine opportunity to improve a patient’s experience of discharge and to support the clinical teams with all aspects of discharge planning. Training will be provided and so direct experience of managing patient discharge is not essential. If you are committed to providing excellent patient care and experience, have great communication and team working skills this could be the post for you.
