We are looking for an experienced Health Visitor who will take on a specialist role and who is ready to take a challenge

We are expanding our Vulnerable family’s team in Coventry and looking for a Specialist Health Visitor who will work with the most complex families in Coventry.

You will need evidence of continued professional development throughout your career thus far, and an understanding of the complexities of multi-agency working which will underpin your practice. Experience of safeguarding and the processes will be vital when working with the most marginalised families in the city.

Presentation skills are essential, evidence of this will be required as this is a key part to the role.

Nurse Prescribing is a fundamental requirement as well as a holistic approach to problem solving.

A non-judgmental and compassionate approach is needed, and the ability to work in a very proactive team is essential which will enable you to succeed in this role.

The aim is to improve health outcomes, reduce inequalities and safeguard children and young people in the locality through the provision of an evidence based and needs led service to those vulnerable groups.

The post holder will take an operational and clinical leadership in the development and delivery of an effective Health Visiting Service with an emphasis on partnership working and integration. The post holder will take responsibility for managing projects and leading on work streams/steering groups and strategy meetings to enable effective delivery of services and service development.

Main Competencies & Responsibilities

To carry out health promotion activities to support the assessed need of this targeted population group.

To facilitate the development of partnership working to encourage multi-agency involvement in improving the health for this population. These may include Local Authority, City Council, Travellers Education Service, Police, Meridian Centre, Anchor Centre, UHCW, Health Visiting Teams across the city and all third sector agencies.

To participate in multidisciplinary meetings, raising awareness about public health and social issues facing this client group.

Perform a health needs analysis of the targeted population. This will involve performing health assessments of this vulnerable population as appropriate, ensuring that all health assessments take into account the child and family’s cultural and religious beliefs.

Develop and establish care programmes in response to the identified for this client group need and provide specialist health promotion advice as appropriate.

As required, provide written reports; attend relevant meetings and /or The Court to share information relating to the safeguarding and protection of children.

Comply with SWFT Safeguarding guidelines, policies and procedures, maintaining contemporaneous record keeping in line with Trust guidelines.

Participate in Clinical Safeguarding Supervision with SWFT Safeguarding Named Nurse, on a quarterly basis.

As a Community Nurse Prescriber, diagnose and prescribe treatment for a range of childhood conditions and minor ailments, within your scope of practice.

Offer and provide ante-natal and post-natal care under the remit of ‘The Healthy Child Programme’ for all children who are identified within this client group including those children who remain with their biological families who are Child in Care, Child in Need or placed on Child Protection plans.

Support and to be an advocate for those families who do not have a GP registration and to encourage families to register asap.

Provide an effective link role with GP practices, in particular specialist GP practices including the Anc
