The post holder will work as a member of the Lotus Team for Vulnerable Patients supporting Mothers to be who are under the age of 19 years old. This team also promotes continuity of care for patients who are socially vulnerable, including those with mental illness, substance misuse and supporting the prevention of pre-term births. There will be opportunities for training and development within these specialist areas for the successful applicant.

The post holder will promote team working to ensure maternity care is responsive to the patient’s needs and by doing so facilitate continuity of care. The post holder will demonstrate the principles and practices of self-management across the team in order to create a service where midwives are able to practice safely and effectively, with a high degree of autonomy and in a highly responsive and personalised way. The post holder must be able to work autonomously, be responsible for managing a caseload and ensure they are referred for appropriate interventions and properly safeguarded.

The post holder will be both hospital and community based and work alongside the Advanced Clinical Practitioner (ACP), Substance and Alcohol Misuse Lead Midwife (SAMS) and Obstetric Lead for Perinatal Mental Health to meet the individualised needs of these vulnerable patients. They will also be required to work closely with external agencies such as the Family Nurse Partnership, Perinatal Mental Health team/RISE and Change Grow Live. The role will include conducting antenatal and postnatal care in clinics and in patient’s homes across Coventry and Rugby.

The post holder will also work with the wider Maternity team to provide intrapartum care, supervise staff, teaching of students, promote and monitor safe and effective clinical practice. The post holder will work towards enhancing the patient/client experience and will be pivotal in the delivery of the Local Maternity’s Systems and National objectives in Continuity of Maternity Care at UHCW.

To set agreed standards of midwifery care, to monitor them and take action to improve where necessary.

Promote an evidenced based, sensitive woman centered approach to care, in collaboration with carers and relevant health care professionals.

Good verbal and written communication skills so that professional advice or care guidance and instructions are understood by women, their relatives or carers, and members of the multi-disciplinary team.

Provide continuity of care for women.

Ensure good discharge planning so that individual women’s needs are met with an appropriate plan of action and involving the multi-professional team as required.

Ensures that all clinical documentation is completed correctly and regularly

Work effectively as part of the wider multi-professional team.

Remain clinically and professionally up-to-date with current trends and developments in the clinical speciality.

Be fully involved in the health education of women and families, taking every opportunity to promote a healthy lifestyle.

Ensure the health, safety and welfare of all staff, women and families in the team, with special reference to incidents and accidents and report them accurately. Take necessary actions to ensure safety is maintained.

Maintain a safe working environment.

Be accountable for the assessment, planning, implementation and evaluation of programs of care within government guidelines and local initiatives.
