Social Prescribing Link Worker

Part time 22.5 hrs

To support a positive shift in power and decision making that enables people to feel informed, have a voice, be heard and be connected to each other and their communities the South Tyneside and North Tyneside PCNs are expanding their teams to include Social Prescribing Link Workers, Health and Wellbeing Coaches and Care Coordinators.

Personalised care means people have choice and control over the way their care is delivered, based on ‘what matters’ to them and their individual strengths, needs and preferences. Personalised care will provide more options, better support and joined up care at the right time. Personalised care has the potential to reduce health inequalities.

We asked the people who have experienced our service what matters to them in a worker. They said someone who is knowledgeable, can talk well, has a fun side, isn't timid, empathetic, not dominating and firm but fair. If you fit their description then we, and them, would love to hear from you

As a Link Worker you will you will work with people to support them to develop their knowledge, skills and confidence to become active participants in looking after their own health. You will guide and support people to reflect on and change their health-related behaviours to help them reach their self-identified health and wellbeing goals. You will introduce or reconnect people to community groups and statutory services and work with local partners to identify gaps in local provision. You will be skilled in coaching, communication and behavioural change skills and are able to work alongside people (individually or in groups) at their starting point to encourage and support them to become more engaged in managing their own health.

You will be expected to work within clinical and community settings. You will support a caseload of people with complex social and emotional. This role will involve working across several teams, so you will be an enthusiastic worker, with excellent communication skills and a proven track record of high quality independent and team work. You will be caring, dedicated, reliable and person-focussed and enjoy working with a wide range of people. You will be highly motivated and proactive with a flexible attitude, keen to work and learn as part of a team, be solution focussed and committed to providing people, their families and carers with high quality support.

Full training will be provided and you will receive ongoing supervision to ensure continuous personal development and support you to reflect on your practice. Our Social Prescribing Link Workers complete our accredited Health Coaching skills programme, creating dual roles that work to agreed competencies.
