**Primary Duties and Areas of Responsibilities**: Communication and Working Relationships Provide support and advice in relation to Parkinsons Disease Undertake nurse-led consultations for Parkinsons (PD) patients and support clinical management of patients. To include telephone F2F and Attend Anywhere. Provide input into patient care packages for potential discharge on wards or other acute areas e.g. ED, community hospital, Placement Liaise with acute and community teams to formulate a management plan to enable the patient to maintain optimal independence and symptom control.

Referral to other staff / agencies as appropriate Promote self-management / educational programmes to patients, carers and professionals Maintain effective communication with the Parkinsons multidisciplinary team Planning and Organisation Manage a clinical case-load. Provision of specialist, high quality, expert nursingcare and support for patients and their carers with a diagnosis of Parkinsons inline with Trust Policies and in liaison with the multidisciplinary team(s) and other health agencies. Facilitating rapid, appropriate assessment, referral to other teams (if required) and discharge To act as a resource for patients relatives and the clinical teams through the provision of comprehensive advice, information, teaching and training with regard to disease management Manage changes in practice in line with national and local developments to enhance the quality of care. Responsibility and Accountability Contribute in audit processes.

Formulate an annual report/clinical audit of own CNS service and outcomes Maintaining and supporting effective communication with all persons associatedwith patient care or support. Active involvement in the development of the service locally and nationally. Assist with the design and facilitation of development programmes in relation to service development Monitor, facilitate and evaluate feedback to develop the service and enhance quality care. Ensure record keeping is accurate and contemporaneous.

Maintain CPD and professional registration To prescribe within the sphere of Parkinsons disease. Policy and Service ResponsibilityUpdated May 2021 Page 5 of 12 Keep up to date with policy developments nationally and locally. Support implementation of new guidance and policies Work with the team to formulate policies/guidelines and standard operating procedures and coordinate appropriate review. Responsibility for Finance, Equipment and Other Resources Whilst no principal budgetary responsibility; as a prescriber, be able to work within the Trusts formulary of medications.

To be familiar with and responsible for IT and other equipment required for the post. Be responsible in the use of all resources Responsibility for Supervision, Leadership and Management Access to your own clinical supervision and ensuring this is recorded. Participate in group supervision through the local PDNS Forum. Lead staff within the Hospital and community regarding best practice forParkinsons disease care.

Lead and participate in implementing research in to practice, through ensuring you keep up to date. In addition, participating in research within this role and linking with the R & D Teams to enable this. Involvement in reviewing medication related incidents in this field (DatixReporting) Lead link staff in all settings ensuring appropriate supervision and education. Information Technology and Administrative Duties To be familiar and competent with common IT systems Word, Excel, Trust systems for; patient letters, Infoflex, PAS/IHCS, Cyberlab and WinDip.
