This is an exciting time to join our passionate, supportive Movement Disorder team which is keen to explore new ways of working to deliver evidenced based, effective person-centred care for our population.

You need to have proven, excellent leadership and interpersonal skills, including the ability to influence and motivate others. Most importantly be as driven as we are about delivering high quality care to those living with Parkinson’s Disease and their families & care givers.

Our Movement Disorder service is hosted within the Healthcare of Older People and Neurology Departments and the service is jointly led by five Consultants; two Geriatricians, two Neurologists and one Physician supported by two Parkinson’s Disease Nurse Specialist Practitioners. Whilst Consultant services are predominantly out-patient based the Nurse Specialist Practitioners provide expert management, support and education across community and in-patient settings; communicating and co-ordinating with health and social care teams with the aim of supporting those living with Parkinson’s Disease to be as independent as possible with a focus on self-management.

- Actively leading service development and participating in implementing change

when required.

**Primary Duties and Areas of Responsibilities**:
Communication and Working Relationships
- Provide support and advice in relation to Parkinson’s Disease
- Undertake nurse-led consultations for Parkinson’s (PD) patients and support clinical management of patients. To include telephone F2F and Attend Anywhere.
- Provide input into patient care packages for potential discharge on wards or other acute areas e.g. ED, community hospital, Placement
- Liaise with acute and community teams to formulate a management plan to enable the patient to maintain optimal independence and symptom control.
- Referral to other staff / agencies as appropriate
- Promote self-management / educational programmes to patients, carers and professionals
- Maintain effective communication with the Parkinson’s multidisciplinary team

Planning and Organisation
- Manage a clinical case-load. Provision of specialist, high quality, expert nursing

care and support for patients and their carer’s with a diagnosis of Parkinson’s in
line with Trust Policies and in liais
