Job summary

An exciting opportunity has arisen for an enthusiastic Band 5 Registered Nurse to join our friendly, supportive and proactive Inflammatory Bowel Disease team. We are based on Hutchings outpatient department, providing care Monday to Friday. Currently able to offer 25hrs a week but may be possible to increase in the future.

You must be committed to providing high quality care to patients. Experience in IBD, gastroenterology or long-term health conditions would be preferable but is not essential as this post offers opportunity for development of knowledge.

You must be willing to get to know patients and their families to enhance their care and be proactive in meeting the needs of the service. It is crucial the successful candidate is able to communicate effectively and work as part of the multidisciplinary team.

Previous gastroenterology experience is preferable, although an induction and orientation will be provided to assist with the learning needs of the post holder and full training/ support will be given

Main duties of the job

Provide patients with up to date, evidence-based information regarding their illness and care Assisting with advising patients via the telephone and email service. Attending network group meetings to help keep up to date with relevant clinical data Attending weekly MDT meetings. Assessing patients' needs in both virtual and face to face settings. Escalating clinical concerns to relevant clinician Assisting Nurse Specialists in their extended roles including with medicines management Assisting with blood monitoring Liaising with research and development team and assisting to identify and recruit patients to clinical trials Undertake nurse led IBD clinic with support from IBD colleagues Data entry into the National IBD registry Although supported, must be able to work without direct supervision

Job responsibilities

The post holder will assist in the delivery of high-quality Inflammatory Bowel Disease (IBD) services throughout the Trust. Working alongside other clinical colleagues to provide expert care, treatment and support for this patient group.

The post holder will contribute to the team in their work to advance the body of knowledge through education, research and audit as well as developing practice. They will play a key role within the multidisciplinary team in order to ensure a seamless, coordinated and timely approach to care.

The post holder will be responsible for supporting the IBD advanced specialist nurses, IBD administrator coordinator and the Gastroenterology team

The role is suitable for an experienced Band 5 Nurse, ideally with relevant experience in Gastroenterology. This post provides the opportunity to further develop communication, teaching and clinical skills within the specialty of IBD. The post holder will be expected to assist with triaging the patient adviceline, monitoring patients blood results and attending the MDT meeting each week and assisting with outcomes is also an integral part of the role.

The post holder will be supported to perform within the IBD team, liaising with patients by telephone and or face to face. Consultation, pre-assessment prior to initiating patient treatment and to improve understanding of the types of medications and treatment offered to IBD patients who have the diagnoses of either Crohns and or Ulcerative colitis. There will be opportunity for service development within the role and be able to support your colleagues to deliver an effective patient centred service.

Participating in recruiting patients into relevant clinical trials and liaising with research and development team will also be an integral element to this role, as well as updating patient databases for audit purposes.

NMC registered nurse in Adult nursing Evidence of continuing personal/professional development Ability to organise and prioritise work load for self and others Able to assess, plan, implement and evaluate patient care IT literate, including data collection and interpretation Good interpersonal and communication skills Evidence of working well within a team but the ability to work independently Ability to work accurately and reliably under pressure

Desirable

Degree Level qualification Mentoring/teaching experience/qualification Experience in gastroenterology Experience of multi-professional collaborative working Experience of giving psychosocial support to complex patients Ability to undertake IBD related assessment. Devise and implement appropriate and safe care plans Venepuncture/ cannulation experience
