A great opportunity has arisen for a Health Care Support Worker to join our community Palliative Respite Team.
The post holder will work as part of the Palliative Care Respite Team and will be responsible to the Team Coordinator. The Post Holder will assist the District Nursing Team in the delivery of high quality community based nursing and social care to patients who have palliative care needs and who wish to remain at home. The post holder will work collaboratively with other members of the Intermediate Tier Services in supporting the patient and carers and provide personal, social, psychological and spiritual care. They regularly work without direct supervision in a patient's home.
Delivers nursing and social care to patients who have a palliative diagnosis and who wish to remain at home under the direction of the palliative care respite team coordinator and under the supervision of the district nurse. This will be in the form of personal care, ie, pressure relief, personal hygiene, continence care, oral hygiene etc.
To provide social support and care, ie to prepare drinks and light meals for the patient if required. To provide a flexible sitting and support service to enable carers to have respite periods and maintain patients within their own homes.
Maintains accurate and up to date documentation.
The post holder will be able to offer psychological support to patients and family/carers. They will possess effective communication skills, recognise individual worries and concerns and respond in an empathetic manner.
Flexibility of working hours to meet the needs of the Palliative Care Respite Team service including some evenings, weekends, Bank Holidays and nights on rotation.
Attends regular team meetings, disseminates information to team members and takes an active role in future team development.
Carries out clerical work within the team as delegated by the team coordinator such as stock ordering and filing.

Communications
Communicates and liaises with the district nursing team and other community services as appropriate and the respite team coordinator, reporting any changes and contributing to the ongoing care planning of individual patient care.

Responsibility for Patient Care
The post holder will deliver appropriate and effective nursing and social care to patients, working under the supervision of the Clinical Nurse Specialists, District Nursing Team and the Team coordinator.
Planning and organising
In the absence of the Team Co-ordinator, the post holder may on occasions be required to assist with the organising of daily work for the team. This would be under the supervision of the team.

To assist the Trust in reducing healthcare acquired infections (HCAl's) all staff should be familiar with all the Trust's Infection Prevention policies which are appropriate to their role. You are required to attend mandatory training in Infection Prevention and be compliant with all measures known to be effective in reducing HCAl’s.
