We are looking for a midwife with an interest in equality, diversity and inclusion to join our community team working specifically with pregnant people who have complex needs or are at higher risk of experiencing poor maternal and/or neonatal outcomes.

The role in community will have a smaller caseload than a traditional community midwife, to enable you to provide a more personalised approach and ensure continuity in the antenatal and postnatal periods.

As well as the multi-disciplinary team working within the Trust, you will also be working with external agencies including health visiting, family nurse partnerships, social care, local volunteer services etc. to co-produce personalised care plans with the pregnant woman / person.

The role includes on call requirement for the homebirth service and unit escalation. This is not currently a full continuity of carer model, but may evolve into this as part of the ESNEFT maternity transformation programme.

To provide safe and effective midwifery care throughout the pregnancy continuum in a variety of settings including children’s centres, hospital and the pregnant person’s home.

Responsibility for a caseload of pregnant people with complex needs, including but not limited to:

- Asylum Seeker / Refugee / Traveller
- Complicated Child Protection Issues
- Vulnerable Teenagers
- Drug / Alcohol Misuse
- History / Current Severe Mental Health Issues
- Non-English Speaking with Complex Needs
- Domestic Abuse
- Learning disabilities
- Female Genital Mutilation

You will work in partnership with a multidisciplinary team including Social Care, Perinatal Mental Health, Safeguarding, Health Visiting, Family Nurses, Consultant Obstetricians, Specialist Services, Voluntary and Community Services.

You will plan and implement the highest standards of individualised and holistic care, advocating for the vulnerable population and addressing challenges that may inhibit people with socially complex lives / learning disabilities / language and culture barriers accessing maternity care.
