Exciting opportunities have arisen following a review of provision of mental health services for older people at West London NHS Trust. We are in the fortunate position of transforming and expanding our service to meet the needs of older people with moderate to severe mental health needs. We are looking for a band 4 Link Workers to join our Older People’s Mental Health (OPMHS) to join our new team. SOAMHS is part of the Older Peoples Mental Health Service (OPMHS) line and has sister teams in three Boroughs, Ealing, Hammersmith and Fulham and Hounslow

Hammersmith and Fulham SOAMHS is a small dynamic and unique team that needs an enthusiastic professional to provide a leading role in offering patients with complex enduring mental health needs and their carers assessments treatment and care.

We are recruiting 1 Band 4 Link Worker for Hammersmith and Fulham catchment area.

This post is ideal for a Link Worker who wants to work with older people in the community who have mental health difficulties. We ensure as an employer that the required support you will be part of a new team and as such will be at the forefront of this pioneering work.

The post holder will work under the supervision higher banded nurse. The post holder will carry a defined caseload of patients. They will utilise their clinical knowledge and skills in the assessment and care of Older Adults with functional mental health problems in the community. They will work within a multi-disciplinary team to undertake the facilitation of groups and individual therapeutic activities.

Hammersmith and Fulham borough has an excellent public transport links therefore having a car or driving is not essential.
