This is an exciting new role that develops collaboration between Independent Care Homes providers and TSDFT. You will be working with care homes and colleagues across health and social care to support people to find the right place for their care.

ou will bring your experience of working with care homes and hospitals to support collaboration and a focus on the person's needs. You will be working independently to ensure that the assessment accurately reflects the person's needs, and that care providers are supported to develop excellent person centered plans.

You will be working alongside the Torbay and South Devon Quality Assurance Teams. The QAIT teams are passionate about working in partnership with the Care Homes and delivering quality referrals. The team is flexible and promotes a healthy work/home life balance.
