An exciting opportunity has is available for a Graduate Mental Health Worker to join our Community Mental Health team. Queens Park and Paddington Community Mental Health Team is a multidisciplinary team made up of nurses, doctors, social workers, peer workers, admin and therapists providing mental health care to the population of North Westminster.

The team works 9-5 Monday to Friday. This role will be to work Wednesday to Friday each week - a total of 22.5 hours

The successful applicant may have contact with patients or service users. As an NHS Trust we strongly encourage and support vaccination as this remains the best way to protect yourself, your family, your colleagues and of course patients and service users when working on our healthcare settings.
- Support users of mental health services in their recovery through shared assessment and provision of psychological and social interventions with multidisciplinary members of the mental health hub
- Participate in the provision of the initial screening, triage and care navigation service of the mental health hub.

CWNL is a large organisation providing services throughout a person’s life, covering community, sexual health, mental health, health and justice and addictions services.

The Queens Park and Paddington (QPP) Community Mental Health team is a large friendly team made up of a range of clinical and non-clinical staff who work together to provide good quality care to residents in North Westminster.

Triage
- Processing and screening referrals received into mental health hub - shared responsibility with Pathway Coordinator
- Developing links with local community services that will contribute to the repertoire of resources available to mental health users in Westminster to enable recovery and where appropriate, signposting service users to local services for support
- Participate in triage/assessment of new patients who have therapy needs
- Supporting weekly Hub therapies meeting - prepping cases for discussion, taking minutes and following up on actions

Psychological Interventions
- Take an active role in supporting the team in the provision of high quality individualised, evidence-based care under the supervision of clinical staff
- Running groups with service users with support from psychologists/Occupational Therapists in the team e.g. Grounding, Sleep Management, Skills, Relaxation etc
- Support senior staff in carrying out day to day tasks associated with provision of care such as written and verbal liaison with service users, carers and other providers of care.
- Carrying out check in/welfare calls with service users who are awaiting therapies treatments in the hub
- Communicate effectively with service users and their families/carers with written care plans or signposting information
- To provide active mental health monitoring of service users and providing feedback on changes in presentation ensuring that marked changes are shared with appropriate professionals in a timely fashion
- Ensuring that professional communications are conducted at the highest standards being effective and efficient.
- Ensuring all relevant information regarding the service user is documented clearly and in a timey manner on SystmOne to provide an accurate audit trail of care and decisions
- Ensure knowledge and skills are at an appropriate level in line with Clinical Governance guidelines, achieving this through supervision, training and development and maintaining a personal development plan with achievable objectives as set with line manager
- Developing a good working knowledge of SystmOne with appropriate training and support.
- Participate in clinical audit and quality improvement (QI) activities
- Undertake any other duties commensurate to the grade and as requested by the line manager
- Attend staff meetings, updates and attend mandatory training as required
